Your First Visit
Schedule
Book your appointment online through our patient portal or call our office directly to speak with our scheduling team.
Complete Paperwork
Arrive 15 minutes early to complete intake forms, or fill them out online before your visit to save time.
Initial Evaluation
Comprehensive assessment lasting 60-90 minutes. We discuss your symptoms, health history, goals, and order necessary lab work.
Treatment Plan
After lab results are in, we create a personalized care plan together based on your unique needs and objectives.
What to Bring
Photo ID
A valid government-issued photo ID. If you plan to use insurance for your lab work, bring your insurance card as well.
Medication List
Complete list of all current medications, supplements, and vitamins including dosages and frequency.
Prior Medical Records
Recent lab results, imaging reports, specialist notes, or any relevant medical records from the past 12 months.
Emergency Contact
Name and phone number of someone we can contact in case of an emergency during your care.
Patient Forms
Forms are available at our office and can be completed at your first appointment. Arriving 15 minutes early is recommended if you haven't filled them out beforehand.

Convenient Virtual Care
Many follow-up appointments can be conducted via secure video conferencing, allowing you to receive expert care from the comfort of your home.
Telehealth is ideal for follow-ups, lab result reviews, treatment adjustments, and check-ins. Initial evaluations are typically in-person for comprehensive assessment.

Office Policies
Cancellation Policy
We require 24-hour notice for appointment cancellations or changes. This allows us to offer your time slot to other patients who need care. Cancellations made with less than 24 hours notice may be subject to a cancellation fee.
Late Arrivals
Please arrive on time for your scheduled appointment. If you arrive more than 15 minutes late, we may need to reschedule your visit to avoid impacting other patients. We understand emergencies happen—please call us if you're running late.
No-Show Policy
Missing an appointment without notice impacts our ability to serve other patients. No-show appointments may result in a fee and could affect future scheduling. If you have concerns about keeping your appointment, please contact us as soon as possible.
Payment & Billing
We are a cash-pay, membership-based practice and do not bill insurance for appointments, memberships, or treatments. Payment is due at the time of service, and we keep pricing transparent so you always know the cost up front. Many patients can use insurance for lab work, and we welcome HSA and FSA payments. Please contact our office with any billing questions.
